Program Description
Patient-Centered Medical Home concept is not new. It has been around for 40+ years originating in 1967 by the American Academy of Pediatrics (AAP) to enhance the care of children with special needs. The concept has expanded with organizations such as the Future of Family Medicine, AAFP, ACP, AAP & AOA. In 2007 the AAFP, ACP, AAP & AOA drafted joint principles of the Patient-Centered Medical Home which the AMA adopted in 2008.
