I feel that for many, many years, we were relatively inefficient and inaccurate in the way we approached abnormal uterine bleeding, especially with blind endometrial biopsies where the results could not be relied on and there was a chance of missed disease, as well as an inability to really treat any abnormality that was discovered. With the introduction of hysteroscopy and then moving hysteroscopy into the office, we have the ability to both diagnose and treat conditions in the office setting, which is obviously less stressful and easier for the patients to handle; it’s very efficient for our practice; it allows us to thoroughly assess the endometrial cavity, treat and remove lesions which are amenable to office hysteroscopy, giving the patient an immediate benefit with a quick office procedure which allows them a very quick recovery, as well as being a very efficient way for us to run our practice.
The newer instrumentations available to us also means that we don’t have to just do a look approach; we can do a see-and-treat approach. So, instrumentation have really increased our ability to remove abnormalities we find, ranging from polyps to retained products, adhesions or fibroids, and therefore allowing us to do a lot more on the treatment side in the office than we used to, saving the patient from going to the OR and saving us from the inefficiencies of doing so.
